Two Rivers Terminal LLC Fined for Safety Violations
A fertilizer manufacturer in Pasco, Washington, Two Rivers Terminal LLC, faces a $394,200 fine after the tragic death of a worker. The Washington State Department of Labor & Industries announced the penalty on Wednesday.
Surveillance footage showed the worker inserting a ladder into an opening on top of a tanker truck, then entering the tank with a garden hose to clean it. The fertilizer residue inside created hydrogen sulfide gas that proved fatal.
At the time of the incident, Two Rivers Terminal was appealing two prior safety citations from 2022 and 2023, totaling $672,320 for 96 serious and 43 general violations. The company is part of L&I’s Severe Violator Enforcement Program due to past infractions. In 2012, a worker was crushed to death, and since 2010, three others have been hospitalized.
